Instructions
1. Se connecter à CareSimple
2. In the Patient tab, select a patient.
3. Dans le dossier du patient, vous pouvez voir à quel programme il est inscrit.
4. To edit the program, click on the Patient Information tab.
5. Click on the pencil icon in the upper corner of the Medical Information section.
6. Use the dropdown menu to select which program you want to associate with your patient. Click on Save to confirm.
Important : selon les règles de la CMS, vous pouvez sélectionner plus d'un programme, mais un seul pour le suivi. Chaque programme de suivi peut être associé au programme CCM. Vous pouvez également sélectionner un programme de coaching.
7. Le patient fait maintenant partie du nouveau programme que vous avez choisi pour lui.
